Implements Phase 1-3 of the AI-code-review rule-set contract requested in #3:

Phase 1 — schema + validation

  • schema/kody-rule.schema.json: JSON Schema for rule files (stable id, category, severity, scope, examples, inheritance)
  • scripts/validate-rules.mjs + npm run validate-rules: rejects malformed/duplicate rules
  • .gitea/workflows/validate-rules.yml: CI validation on every PR
  • Migrated all 25 existing rules to the new schema

Phase 2 — docs

  • docs/SEVERITY.md: severity model + REQUEST_CHANGES/COMMENT/APPROVE verdict mapping
  • docs/FINDING_FORMAT.md: canonical GitHub-inline-comment finding shape, fail-closed rules
  • docs/CONTEXT_CONTRACT.md: precedence for repo instructions/overrides/ignored paths
  • CONTRIBUTING.md: rule-authoring workflow

Phase 3 — new high-value rules
SSRF, pagination/index bounds, insecure security fallbacks, timing attacks, case-normalization bypass, TOCTOU, tenant isolation, sensitive-data exposure, lifecycle cleanup, transaction/idempotency.

Not yet done (tracked as follow-up): rule-set version manifest/pinning, fixture-based tests.

Implements Phase 1-3 of the AI-code-review rule-set contract requested in #3: **Phase 1 — schema + validation** - `schema/kody-rule.schema.json`: JSON Schema for rule files (stable id, category, severity, scope, examples, inheritance) - `scripts/validate-rules.mjs` + `npm run validate-rules`: rejects malformed/duplicate rules - `.gitea/workflows/validate-rules.yml`: CI validation on every PR - Migrated all 25 existing rules to the new schema **Phase 2 — docs** - `docs/SEVERITY.md`: severity model + REQUEST_CHANGES/COMMENT/APPROVE verdict mapping - `docs/FINDING_FORMAT.md`: canonical GitHub-inline-comment finding shape, fail-closed rules - `docs/CONTEXT_CONTRACT.md`: precedence for repo instructions/overrides/ignored paths - `CONTRIBUTING.md`: rule-authoring workflow **Phase 3 — new high-value rules** SSRF, pagination/index bounds, insecure security fallbacks, timing attacks, case-normalization bypass, TOCTOU, tenant isolation, sensitive-data exposure, lifecycle cleanup, transaction/idempotency. **Not yet done** (tracked as follow-up): rule-set version manifest/pinning, fixture-based tests. Refs #3.
- schema/kody-rule.schema.json: JSON Schema for .kody-rules/review/*.yml
files (stable id, category, severity, scope, examples, inheritance).
- scripts/validate-rules.mjs: validates every rule against the schema and
checks duplicate ids/titles and style/severity mismatches.
- .gitea/workflows/validate-rules.yml: runs the validator in CI.
- Migrate all 25 existing rules: add stable id + category, drop the
redundant `paths` field in favor of brace-glob `path`, and fix
  write-a-clear-scoped-pr-title to be low-severity/style (was
  high/file, blocking on a style-only rule).

Refs #3.
- docs/SEVERITY.md: critical/high/medium/low definitions and the
REQUEST_CHANGES/COMMENT/APPROVE verdict mapping; style rules can
  never block.
- docs/FINDING_FORMAT.md: canonical GitHub-inline-comment-ready finding
shape, location rules (no fabricated locations), and fail-closed
  behavior for security/auth uncertainty.
- docs/CONTEXT_CONTRACT.md: what project-supplied context (repo
instructions, linked repos, severity overrides, ignored paths) can
  contain and its precedence order.
- CONTRIBUTING.md: workflow for adding/editing rules, choosing
severity, writing examples, and reviewing rule changes.

Refs #3.
New rules covering gaps explicitly called out in the issue:
- SSRF: unvalidated user-controlled URLs in outbound requests
- Pagination/offset/limit/index validation, including negative values
- Insecure security fallbacks (fail-fast on missing secrets)
- Timing attacks (constant-time secret comparison)
- Case-normalization bypass (inconsistent normalization in comparisons)
- TOCTOU / check-then-act race conditions
- Tenant isolation (scope queries by tenant, not just resource ID)
- Sensitive data exposure in logs/errors/responses/telemetry/URLs
- Resource cleanup on all lifecycle exit paths
- Transaction boundaries and idempotency for retried operations

All validate against schema/kody-rule.schema.json.

- MANIFEST.json + schema/manifest.schema.json: pinned commit SHA,
ruleSetVersion, schemaVersion, and ruleCount for consumers to verify.
- scripts/generate-manifest.mjs: regenerates MANIFEST.json from git HEAD
  and the on-disk rule count.
- docs/VERSIONING.md: compatibility rules (patch/minor/major), pinning
guidance, and fail-closed behavior when the source is unavailable or
  the manifest doesn't match.
- test/rules.test.mjs: fixture-based structural tests — schema
validity, positive/negative example coverage, no duplicate
ids/titles, style rules never blocking, MANIFEST.json ruleCount
matches disk. Wired into `npm test` and CI.

Warning — unsafe positive example. Resolving the hostname with socket.gethostbyname() and then calling requests.get(url) performs two separate resolutions; DNS rebinding can make the request resolve to a private address after the check. The example also omits comprehensive IPv6/private-range handling. Since this is marked isCorrect: true, it teaches an unsafe SSRF defense. Use a design that validates and pins the actual destination IP (including IPv4/IPv6), and enforce redirect handling/revalidation.

Warning — stale provenance. generatedFrom.commit points to 72f78c2a280470c56500b21c6453b9cee3c8eac0, while this PR head is 8b0b6bd561cea063c41f3887ac212daeb1f16b27. Consumers following the documented manifest contract can therefore reject this checkout or associate it with the wrong rule-set contents. Addresses PR #4 review: the previous example resolved the hostname
with socket.gethostbyname(), validated that IP, then let requests.get()
re-resolve the hostname independently -- a DNS-rebinding gap between
the check and the actual connection. It also only checked
is_private/is_loopback, missing IPv6 and link-local ranges.

Fix MANIFEST.json provenance design and add CI drift check
Some checks failed
Validate rules / validate (pull_request) Has been cancelled

Addresses PR #4 review: generatedFrom.commit can never equal the SHA
of the commit that contains the manifest itself, since committing the
file changes the hash. Reframe it as informational parent-commit
provenance, not a trust anchor, and document that consumers must pin
by git tag/SHA instead (docs/VERSIONING.md).

Add `generate-manifest.mjs --check` (npm run check-manifest) so CI
fails if ruleCount drifts from the on-disk rule files, wired into
.gitea/workflows/validate-rules.yml.

Addressed both review comments:

  1. SSRF positive example (fixed in 0f08cc7): the previous example resolved the hostname once for validation then let requests.get() re-resolve it independently — a DNS-rebinding gap. Now resolves via getaddrinfo (covers IPv4+IPv6), validates all returned addresses against private/loopback/link-local/reserved ranges, and pins the connection to the validated IP rather than re-resolving.

  2. Stale manifest provenance (fixed in 6920877): this pointed at a real design flaw, not just staleness — a committed file can never record the SHA of the commit that contains it. Reworked the contract: generatedFrom.commit is now documented as informational-only (parent-commit provenance), and docs/VERSIONING.md makes git tags/SHAs the actual pinning/trust mechanism. Also added npm run check-manifest, wired into CI, to fail the build if ruleCount drifts from the on-disk rules.

All validation/tests pass (35 rules, 6/6 fixture tests, manifest check green).

Warning — the positive example is still unsafe. Although safe_ip is computed and validated, requests.get(url) still connects using the hostname; the comment only says to implement a custom adapter/transport “e.g.” and no such pinning is present in the example. The client can therefore re-resolve the hostname and remain vulnerable to DNS rebinding. Make the example use an actual pinned transport/adapter implementation (and preserve the correct TLS/SNI behavior), or do not mark it isCorrect: true.

Warning — CI does not enforce the documented manifest contract. docs/VERSIONING.md says CI fails when ruleSetVersion, schemaVersion, or ruleCount differs from regeneration, but --check compares only manifest.ruleCount (and never validates either version field). A drifted schema/rule-set version will therefore pass CI. Either implement checks for all documented fields or narrow the documentation to the behavior actually enforced.

Addresses second round of PR #4 review:

1. The SSRF positive example computed and validated safe_ip but then
called requests.get(url), which still re-resolves the hostname --
the comment describing a custom adapter was not actual code. Replace
it with a real requests.Session + HTTPAdapter subclass that overrides
urllib3's ConnectionCls to dial the pinned IP while preserving the
   Host header and TLS SNI.

2. docs/VERSIONING.md claimed CI checks ruleSetVersion/schemaVersion/
ruleCount, but generate-manifest.mjs --check only compared
ruleCount. Add a real schemaVersion check (sourced from a
   "schemaVersion: N" $comment in schema/kody-rule.schema.json, the
single source of truth for both files) and narrow the docs to
state that ruleSetVersion is intentionally not drift-checked (it's
a manual SemVer decision with no derivable ground truth).

Also wrap the schema/manifest JSON.parse calls with a rethrow that
names the failing file, improving CLI error messages on the (expected
never to occur in a valid checkout) case of a corrupted checked-in
JSON file.

Addressed the second round of review comments:

  1. SSRF example still not pinned (fixed in 45fe999): replaced the comment-only "e.g. via a custom adapter" with an actual requests.Session + HTTPAdapter subclass that overrides urllib3's ConnectionCls._dns_host to dial the validated IP directly, while leaving self.host untouched so the Host header and TLS SNI still use the original hostname.

  2. Manifest check didn't enforce the documented contract (fixed in 45fe999): added a real schemaVersion check to generate-manifest.mjs --check, sourced from a "schemaVersion: N" $comment in schema/kody-rule.schema.json (the single source of truth both files read from). Narrowed docs/VERSIONING.md to state that ruleSetVersion is intentionally not drift-checked — it's a manually-governed SemVer decision with no derivable ground truth to check it against, only schema-validated as a well-formed x.y.z string.

All validation/tests pass (35 rules, 6/6 fixture tests, manifest check green).
